Referral from teacher, staff, parents, and/or outside professionals

· Attended by parents, teacher, staff, student, administrator, and/or outside professionals
· Purpose of meeting is to inform and brainstorm ideas to help the student’s learning process

Obtain further information
Create classroom modifications that meet student's needs
Counseling Support
504 eligibility determination based on
documented physical or mental handicap adversely affecting the student to a significant degree
Special Education evaluation through SELPA Referral Form
Outside referral

Range of Services:
RTI, 504, IEP
consult, push-in, pull-out,
reviewed annually at minimum
